Mentorship Program Consent Form:
1. Voluntary Participation:
I understand that participation in the Mentorship Program is entirely voluntary. I affirm my commitment to being involved for a duration of six months. After 6 months, the official withdrawal from the program takes place.

I acknowledge that being a mentor requires a commitment of time and effort. I am committed to actively participating in the program, including regular meetings with my mentee and attending any training sessions or workshops provided.
3. Confidentiality:
I agree to maintain the confidentiality of all information shared during the mentorship relationship. I understand that trust is crucial for the success of the program, and I will not disclose any personal or sensitive information about my mentee without their explicit consent.

I commit to fostering an inclusive and respectful environment for my mentee. I will treat them with dignity, value their perspectives, and create a space where they feel comfortable expressing themselves.

I understand the importance of maintaining professional boundaries in the mentorship relationship. I will not engage in any behavior that could be perceived as inappropriate or that goes beyond the scope of a mentorship relationship.

7. Communication:
I commit to open and honest communication with both my mentee and program coordinators. If any issues arise, I will promptly address them and seek assistance if needed.
